Applicants ages 16 and older must provide identification verifying current address. Address may be verified through one of the following options:

  1. Illinois drivers license with current address.
  2. Illinois Identification card with current address.
  3. Photo I.D. and current mail addressed to the applicant.
  4. Current utility bill and current mail addressed to applicant.

Applicants age 15 and under must provide identification verifying Parent’s/Legal Guardian’s address. Juvenile must be present in the library for the registration.

I agree to accept responsibility for all materials borrowed on this card. I agree to obey all the rules and regulations of the library, to promptly pay all fines charged against me, and to give immediate notice of any change of address.

Applicants should visit the library to pick up their card within 3-7 days. Applicant will need to show identification and proof of address. For minors age 15 and under, the parent/guardian will need to also be present. Applicant will need to sign the application form at the library and then they will receive their card.
